LIFT Forms in DMAC
Use Formworks to create LIFT forms via common locked templates. District Administrators can grant access to individual forms.
Helpful Tips
- LIFT Observation Forms are available in FormWorks > Templates as locked Common Templates.
- Math, RLA, and Content-Agnostic versions are available, and access must be granted separately for each form.
- Only District Administrators can manage access to these templates.
Granting Access
- Users must have a current district- or campus-level DMAC account.
- Find the LIFT template, select the gear icon > Add Users > Select, and add the appropriate users.
- Only users added to the form can complete observations or generate reports for that form.
Before an Observation
- Teachers must have their State Account and TSDS number registered in DMAC before they can receive an observation.
- If a teacher appears inactive or is greyed out, their State Account may not be registered.
Completing an Observation
- Go to Forms > New Form, search for LIFT, and select the green plus icon beside the appropriate form.
- Select the campus and teacher, then Save to populate the teacher's information.
- Enter the observation date, observer role, grade level, classroom type, and evaluation type.
- For the Content-Agnostic form, also select the content area.

Choose Standard or Calibration:
- Both function the same within FormWorks.
- Calibration observations allow LIFT teams to conduct calibration walks without the data officially going to TEA.
- Enter observation notes and complete the ratings for each section. Companion guide information is available to help determine ratings.
Action Steps
- For a first observation, leave the Previous Action Step as None with no date.
- For later observations, enter the previous action step and completion date.
- Add Praise and Areas of Growth.
- Always complete the Next Action Step and include a due date.
Saving & Submitting
- Select Save and name the form to save it as a draft that can still be edited.
- Select Complete for Submission when the observation is final.
- Once submitted, the form is locked and cannot be edited.
- Completed observation forms are visible only to the observer, not the teacher.
Reports
- Designated FormWorks Authors can access LIFT reports.
- Under Reports, select the LIFT form from the Template dropdown and choose the fields to include.
- Use Print to view aggregated results in a visual format.
- Export to CSV to review detailed data, including previous action steps before conducting another classroom observation.
In this video, you will learn how to enter LIFT Observation Forms within FormWorks.
We'll cover where to find them, how to grant access, how to complete an observation, and how to pull reports.
The LIFT forms are located in FormWorks under Templates. They are saved as Common Templates available for any school to utilize.
Only District Administrators can access these locked templates and provide access to others.
To find them, type LIFT in the search box. Or, if you don't have many forms, they may already be at the top of your list.
They're titled the same way for each school, with Math, RLA, and Agnostic versions available. Access has to be provided to each one.
To let people in your district use these forms, they first need a current DMAC account — at either the campus or district level.
On the form, click the gear icon, choose Add Users, and click Select.
With locked forms, only users added here can complete this specific form or generate reports from data collected.
Once added, other users can login to DMAC, access FormWorks and select the Forms section from the top navigation bar. Depending on the user's access, they may only see Forms at the top or they could have other options such as Templates and Reports.
One important note before observations begin: for a teacher to be eligible to receive an observation feedback form, they must have their state account, including their TSDS number, registered in DMAC.
If a teacher isn't showing up as active, this is usually why.

Now, let's complete a form as an Observer.
After selecting the Forms section, click the New Form button on the top right, find your LIFT form quickly by typing LIFT in the search box — and click the green plus sign to the right of it.
Click Select to pull up the campuses and teachers assigned to your district, choose your teacher, and Save.
That pulls all of the teacher's information into the form. Remember, if the name is greyed out, they do not have their State Account registered.
Next, set the observation date.
Your observer name populates automatically.
Choose your observer role, grade level, and — on the content-agnostic form — the content area.
On the R-L-A form the content area is always Reading, and on the Math form it's always Math.
Pick the classroom type, then the evaluation type: Standard or Calibration.
Both behave exactly the same in the software.
Calibration is there so the LIFT teams can run calibration walks and use that data without it officially going to T-E-A.
Type your observation notes right on the screen.
As you scroll, each section shows its heading, the question that drives it, its alignment to T-TESS, and the rating you can give.
The companion guide information sits just below the routines section to help guide each rating.
At the bottom, handle the action steps.
If it's your first time in the classroom, leave the previous action step as None and don't add a date.
Otherwise, name the previous step and the date it was completed.
Add your praise, copy your areas of growth from the action step guide, and always fill in the next action step with a due date.
That field should be completed every time.
When you're done, click Save and name the form.
Right now, Save works as a draft, so you can go back in and edit it.
When you select Complete for Submission and save, the form locks and changes can no longer be made.
These forms are visible only to the person who completed the observation — not to the teacher.
If you're a designated Author in FormWorks, you have access to Reports.
Choose your LIFT form from the Template drop-down. Select the information you want on the report, including who the forms were completed by and the reporting fields. You can see the data in a more visual format by clicking the Print icon.
Here, the print version shows aggregated totals.
If we go back and click the option to export CSV, this option is especially handy before a classroom visit: you can see the previous action step assigned to a teacher and decide whether it still stands or a new one is needed.
